To apply for a “Small Grant” (REQUESTS UNDER $5,001)
Required:
- One page letter written on organization letterhead describing how funds will be used
- Project Budget and year end financial statements, if available
- 501 (c) (3) letter of determination from the IRS
Deadline:
No deadline. Reviewed on rolling basis. Applicants will receive a response by mail whether funded or declined.
To apply for a “Large Grant” (REQUESTS OVER $5,000)
Required:
In five pages or less, please answer the following questions…
1. Why is this project needed in our community?
2. Tell us about your organization’s history and origins.
3. Who is your constituency?
4. What are your current programs and accomplishments?
5. Please describe the qualifications of each management staff member responsible for the success of this project.
6. If funded, how will you evaluate the success of this project within one year?
Please attach the following financial and additional information:
1. Project Budget, including funding already committed or pending
2. Current Board of Directors listing, with professional affiliations
3. A statement outlining the percentage of Board contribution to the project and the total dollar amount provided by your Board of Directors in the current fiscal year
4. Board-approved operating budget for current fiscal year
5. A statement including the percentage of Board participation for the current fiscal year or project, along with the total amount given by the Board collectively
6. Your most recent annual report, audited, if available
7. Copy of 501 ( c ) ( 3 ) IRS letter of determination
8. Up to three examples of letters of support and / or press related to your project or organization (optional)
Deadline:
For First Quarter Review: February 28th
For Second Quarter Review: May 31st
For Third Quarter Review: August 31st
For Fourth Quarter Review: November 30th
